Police! Duplicate images are uploaded!

What I did:
I see duplicate images being uploaded!
What I expected to happen:
The first occurrence of an image upload should be the only copy; any subsequent uploads of the same image should result in a "saved" version of the original to a users shake. A similarity test should be run after uploading, prior to posting (checksum/hash or better) and the right thing should happen after. Additional filename, source, comments, etc. should be recorded with the original post as a comment (optionally?)
