Possible to perform multiple likes on the same file?

Not sure how it happened, but it may be possible to perform multiple likes on the same file.

- State changed from “new” to “hold”
While I couldn't replicate it. It's there in the DB. I'm about 50% sure it is related to a db issue that was going on yesterday where requests were backing up. What might have happened is the two requests were issued simultaneously. If I see another one of these again I will definitely look into trying to trap it, but after inspecting the code we can't see how it could have happened in software.
I'll just put this on hold to remind myself of it and the URL.
